Planning for the Unexpected
The S.M.A.R.T. Care Program provides additional sharing for those faced with life-threatening illnesses such as Stroke, Heart Attack, Aggressive Cancer, Renal (Kidney) Failure, and major Organ Transplants.
In the event of these medical conditions, the S.M.A.R.T. Care program will share for extra medical and out-of-pocket expenses related to the illness, including childcare, paycheck protection, and other essentials; even mortgage and rent.
With S.M.A.R.T. Care by UHSM, there are no upfront application fees, and sharing can begin 30 days after diagnosis to help cover unexpected expenses quickly.
Offering catastrophic healthcare support, the program helps members lessen the financial burden commonly caused by the unexpected costs of a serious illness. This includes support with bills, such as mortgage, rent, utilities, and over 10 other related expenses. Plus, your spouse and dependents are partially eligible for sharing without any additional contributions.

If diagnosed or being treated for a qualifying illness, you will be eligible to access 100% of your selected share amount, up to $50,000.

While we encourage you to leverage the negotiated rates with the PHCS® PPO network for medical services, S.M.A.R.T. Care is available for qualifying illnesses regardless of the care provider seen.

S.M.A.R.T. Care
The S.M.A.R.T. Care program provides additional sharing for individuals facing major life-threatening illnesses such as stroke, heart attack, aggressive cancer, renal (kidney) failure, and major organ transplants.
As a catastrophic healthcare solution, this program ensures that members receive additional financial support to help cover both medical and out-of-pocket expenses. This includes essential costs such as mortgage or rent payments, childcare expenses, and paycheck protection, offering financial stability during treatment and recovery.
The program also helps cover auto payments, insurance for health, home, and vehicles, as well as utilities. In addition, it provides support for mental and family counseling, emergency transportation, extended care, and afterlife expenses, offering comprehensive assistance to help members focus on their health without financial strain.
A.I.D.D. Care
The A.I.D.D. Care program shares the costs associated with qualifying accidents, including any unmet AMCS amounts from core UHSM health share programs. It provides financial assistance for immediate medical care required following an accident, as long as treatment is obtained within 48 hours.
Additionally, it covers ongoing care typically received within 30 days of the accident. As part of our catastrophic healthcare support, this program helps with expenses related to transportation, ambulance services, emergency room and urgent care visits, anesthesia, lab tests, radiology, specialized therapy treatments, surgery, hospitalizations, prescriptions, and in-home recovery care.
If an accident results in the loss of a limb, the program also shares the costs of prosthetics or necessary home modifications. In the most severe cases, where an accident leads to death, any remaining funds are distributed to the primary or designated beneficiary, providing financial relief to loved ones during a difficult time.
